Get a Library Card |
Dane County Library Cards are issued to residents of any town, the villages of Blue Mounds, Brooklyn, Cottage Grove, Dane, Maple Bluff, Rockdale, Shorewood Hills, and the City of Fitchburg. Cards may be obtained through any public library, including the Bookmobile, in Dane County. They are issued free, but do require a form of identification which includes a current address, such as a driver's license or passport. Another form of identification may be acceptable if it is accompanied by a piece of mail received at your address within the last month. Cards are issued to individuals of any age (a parent's or guardian's signature is required for those under 14), or to organizations or businesses. Ask at the Circulation Desk for a library card application form, or print an application below (you must have Adobe Acrobat installed), complete it, and take it to your closest public library or bookmobile stop. Individuals, regardless of where they live, need only one library card to use any public library in Dane or the surrounding counties. Once issued, a Dane County Library Card will be honored at any area public library; however, local borrowing rules and policies apply. Lost or stolen cards should be reported immediately to any public library in Dane County. Cards can be replaced for a minimal charge at any public library. |
